Pre-qualification Criteria
All supplies wishing to be considered for Pre-qualification should include the following
requirements in their bids/proposals:

  1. Business profile including contact address and details of Key staff
  2. Business registration certificate/ Business license
  3. List of the current customers to the business with their contact details and at least written
    references of endorsement from three key/ reputable organizations
  4. Copy of Suppliers valid MRA Tax Clearance certificate and VAT Certificates
  5. Withholding Tax Exception Certificate
  6. Copy of passport and IDs for Company board of Directors
  7. Proof of residence i.e. Utility bills for not more than 3 Months
  8. Proof of Bank account, bank statement not later than 3 months
  9. Registration Certificate with the Office of the Director of Public Procurement and Disposal
    of Asset (PPDA)
  10. Proposed delivery and payment Terms
  11. Market value/cost structure for the goods and services
  12. Evidence of financial capacity (recent Audited Accounts/Recent Bank Statements)
  13. Available of key personal and proof of competence if applying for the provision of services
  14. Physical address and all other contact details (postal, email, phone and fax)
  15. Other information relevant to back up the application

NEEDS ASSESSMENT FOR COVID-19 RESPONSE PROJECT 001

1.0. INTRODUCTION
The needs assessment will typically collect qualitative and/or quantitative data; three weeks within commencement of the project activities in order to track and assess both software and hardware interventions and materials needed to effectively respond to Covid-19 pandemic in 40 school communities, market spaces and border posts in Malawi, Lesotho, Zambia and Zimbabwe.
1.1 BACKGROUND
Global Hope Mobilization (GLOHOMO) has been given a grant to implement an eight months (May to December 2020) emergency response project to Covid-19 pandemic. The package for this emergency response include implementation of interventions which will effectively address key challenges affecting health and education sector amidst Covid-19 in Malawi, Lesotho, Zambia and Zimbabwe. The overall goal of this emergency project is implement innovative, tailor made and sustainable interventions to respond to Covid-19 and even the future similar pandemic.
2.0. OVERVIEW OF THE SURVEY
The main purpose of this Needs Assessment will be collection of baseline data that will serve the purpose of informing the project team on the situation on the ground. At the end of the project, a decision on whether the project has achieved its targets, goals and objectives will be made based on comparison between the results of the needs assessment and end of project evaluation report. The Needs Assessment shall also inform the kind and magnitude of interventions to be employed per specific country or site as level of preparedness may vary from one country or site to another. The needs assessment shall also include the use of different data collection tools to be used for Key Informant Interviews (KII), in depths interviews (IDI) and a facility questionnaire (a special questionnaire to capture baseline or status data of sample schools and health facilities under Future Life-Now!)
